1. All fields in PART 1 of the form MUST be duly filled up.
2. For bank account holder who is using thumbprint, please get your banker to verify before mailing the form.
3. All amendments on the form MUST be counter-sign.
4. Please mail the original copy to:

5. You may also submit the form to our Customer Service Kiosk at the car park or at our head office.
6. Please note that banker require 1 working month to process GIRO application.
7. We will mail a confirmation letter once your application is approved.
